Erica Guimaraes is a multi-faceted public health professional and Massachusetts state certified Community Health Worker (CHW) with decades of lived experience and expertise in the design, implementation, technology integration and management of CHW-led programs, resulting in improved quality and utilization within medically complex, culturally diverse populations with high social determinants needs. Ms. Guimaraes developed comprehensive chronic disease programs and managed a research initiative across 15 primary care practices of Massachusetts General Hospital. At the Massachusetts Department of Public Health (MDPH), Erica collaborated with local, state and national partners on key policy issues pertaining to an integrated and sustainable CHW workforce. As well, she supported MA CHW certification and provided technical assistance to MassHealth (MA Medicaid), healthcare organizations and public health departments who sought to improve their CHW infrastructure. Along with designing and coordinating CDC grant projects, Erica helped in planning annual statewide CHW conferences, sat on the Advisory Board for the Massachusetts Association of CHWs and collaborated with the MDPH Office of Statistics and Evaluation on multi-year, statewide surveillance data on CHWs and their employers.
Presently, Ms. Guimaraes oversees expansion of social determinants of health screening, implementation of remote patient monitoring and extension of CHW workforce at 22 primary care sites of the Mass General Brigham health system in Greater Boston. Erica holds a Bachelor’s degree in Psychology from Gordon College and in May 2022, will complete an Executive Master of Public Health degree at Boston University.
