  • Provides group facilitation, leadership development, performance coaching, cross-cultural communication and coalition-building

  • Special focus:  Immigrant & refugee communities, First Nations; substance use and mental health

MARILYN GARDNER, RN-BS

  • Provides cross cultural communication, group facilitation, leadership development, individual coaching and public health program building

  • Special focus:  Immigrant & refugee communities and First Nations; public health and healthcare

  • Provides technical assistance for healthcare organizations, public health, community based organizations and payers on design, implementation and management of CHW-led programs, including developing protocols, addressing social determinants and launching chronic disease-specific initiatives across one or more entities.

  • Assists in program design, implementation and evaluation from health outcomes to cost efficiency at the entity, town, county and state levels

  • Special focus:  Rural, state and local government entities

CONNECTED COMMUNITY HEALTH

  • Advises on strategic planning, program design/implementation and/or technology integration for CHW models within and across healthcare, public health and/or academia, including those addressing social determinants

  • Assists with project management

  • Special focus:  High disparities African American communities especially in the Southeastern states
